Hybrid endovascular repair of Kommerell diverticulum and aberrant right subclavian artery in a patient with repaired coarctation of the aorta

چکیده
A 65-year-old Caucasian man was admitted to the Cardiothoracic Department for elective coronary artery bypass graft (CABG). At pre-operative bidimensional transthoracic echocardiography (TTE), a narrowing of the ascending aorta was observed: the inner lumen was reduced by a circumferential, bulky ridge of tissue projecting from the aortic wall and causing a mild supravalvular stenosis (mean PG 20 mmHg) (Figure 1, A-B). An electrocardiogram-gated cardiac computed-tomography (CT) anIdiopathic ascending aortitis as a rare cause of supravalvular aortic stenosis

Right-sided aortic arches are rare, affecting approximately 0.1% of the population. They are a result of abnormal development of the primitive aortic arches and may present later in life with later life with aneurysmal expansion of the aberrant left subclavian artery 'Kommerell's diverticulum'. These can be challenging to treat effectively. We report a rare case presenting with mild dysphagia a...
متن کاملSuccessful surgical exclusion of rapidly expanding kommerell diverticulum following a total arch replacement for an acute type a aortic dissection.
A 50-year-old man presented with an acute type A aortic dissection with an aberrant right subclavian artery. Emergent total arch replacement with an elephant trunk was performed. Intraoperatively, the origin of the aberrant right subclavian artery could not be resected because it was located too far from the distal arch. After two weeks, the patient became aware of dysphagia. Postoperative comp...
متن کاملSingle Stage Aortic Arch Replacement without Circulatory Arrest.
A 78-year-old man with a Kommerell diverticulum and aberrant right subclavian artery was admitted for thoracic pain and severe malnutrition due to esophageal compression. We performed an atypical surgical procedure including extra-anatomical debranching and direct aortic repair, trying to avoid deep hypothermic circulatory arrest and shorten the cardiopulmonary bypass time.
متن کاملRight aortic arch related to Kommerell diverticulum and internal carotid artery agenesis.
A 31-year-old woman presented with blood pressure somewhat lower on the left compared with the right arm, hyposthenia, and left arm claudication. A vascular ultrasound examination demonstrated the absence of internal left carotid associated with left subclavian artery stenosis. A multidetector computed tomography using a 64-row scanner (Figure 1) confirmed the echographic results and revealed t...
